All forms may be completed, saved on your computer, and emailed in OR printed out and sent in standard mail.

Re Enrollment Fees:

$20 per student Spring—June 20

$30 per student June 21—August 31

$40 per student after August 31

 

Complete one per student for Elementary ( K—6th grade) or Jr/Sr High (7th—12th) level. Turn in one and keep a teacher copy.

For Jr/Sr (7th—12th grades) One page per course (subject). Each student should

have 6 or 7 completed Course Description forms unless approved in advance. Keep a copy for your records

One per student.  Update family information on separate sheet. Both parents must sign.

Enrollment Agreement

One parent must sign and read thoroughly.

Teacher Qualification

Items listed below are due at the Welcome Back Meeting

Tuition:

$ 30.00 per month one student,

$ 45 for 2 students,

$ 55 for 3 students

with a maximum of $65 per family.

Tuition is paid on the 1st of the month, September through June.
